  4. c)Selection of chronic diseases with an overall prevalence of n ≥2% (ordered by prevalence).
  5. d)At least one of the six chronic diseases listed above.
  6. e)Any chronic disease (these six plus other chronic diseases such as arthrosis, asthma, dermatosis or gastro-intestinal disease).
