  1. *Non-parametric test for trend across ordered groups developed by Cuzick [47]. We used this to test for differences between 6 and 12 months in responses for each EQ-5D-5L item
  2. **At 12 months, 226 participants self-reported the EQ-5D-5L in full; 15 provided no response to 5L, 14 had 5L proxy reported by next of kin; 1 partially reported the 5L by responding only to the 5L usual activities, pain/discomfort and anxiety/depression items
