Self-reported hypersensitivity, allergy or intolerance to food or other defined environmental substances.

Self-reported asthma, wheezing or whistling in the chest.

Self-reported recurrent eczema or itchy rash for at least six months.

Self-reported sneezing, runny nose, blocked nose or itchy-watery eyes without a cold.
